Clear specifications agreed by management and the customer(s).

I can summarize them in few points:

• No enough committment of working team (yours or customers') to fulfil project goals
• External project dependencies (hardware, material, other systems, resources) are not there and no one is as keen as you are to resolve the problem
• Unmatched performance/expectations among all stakeholders (some sees performance as accepted, others not)
• Inaccurate information regarding cost and quality leaving no chance for improvement efforts

External depedencies of3rd parties such as Goverments authorities, consultants, subcontractors, and suppliers.
