  2. DONOT take quick actions before evaluating the scenario 

  3.  AVOID pinpointing to an employee and throw the whole issue on somebody`s shoulder "this shows a lack of management and leadership skills

  4. Call the team "Stakeholders" for an urgent quick meeting 

  5. Define the Problem and identify the root cause 

  6. Open/Free discussions & brainstorm is required by all stakeholders to eliminate the cause 

  7. If you have a non-conformity system , document the problem and how you eliminated the root causes

  8. spread the non-conformance report to the concerned personnel , in order to avoid the same mistake to happen again in other sites / with other teams 

     

    Spread the QUALITY Culture .which minimizes your risks and cuts the costs
