It is the responsiblity of both the sender and receiver to ensure the message is not wrongly interpreted.

Sender should confirm wtih the receiver that the message delivered is understood by the Receiver.

Receiver should also repeat or paraphrase the message to the Sender and to confirm if that is what the Sender intended.

In my opinion its project manager's fault because he is the one who makes "Communication Management Plan". Its his ineffeciency that is adding cost to the project.

Terminology related to subject is most important. Sentence phrase should not consist complicated words. Re-confirmation of the msg sense is always essential.

It is the responsiblity of both parties involved but if one party out of two is Vendor or Service Provide then he should be more careful. Verbal communication should be converted to written & approved communication to avoid miscommunication at maximum extent.

All messages should be interpreted in the context of work that is between the sender and reciever and should also be as per the contractual terms. If a msg is not understood or its interpretation is not being understood, clarification should be sought before acting on the message. It is also good to have a comunicatoon matrix agreed between the two parties, so that msgs are sent to the right person. Another protocol would be to send to more than one recipient amongst both the parties so that miscommunication can be flagged by somebody.
