basic causes is the direct contributing factor to the incident, a basic reason that motivate people do the unsafe acts or make an unsafe condition
root causes is the factor behind the contributing factor, usually it already exist for a long time but since it did not directly make an incident/ near miss, people did not aware with the consequences
systemic cause is one of the last category of the root cause. sometimes the system is correct but the doer (poople) do wrong/mistakes. or even system and people already correct but the equipment (physical/ material) that bad.
systemic cause can only be known after review if the policy, guideline, procedures etc is potentially contribute to the incident/ near miss
