HR usually determines the frequency of the appraisal process.  Many organizations time this process to be an "annual" process. i.e. done once per year and they tend to tie annual salary increases, promotions, other rewards, career advances, training, skills development to be aligned with the outcomes of this process.

There might be other "special" needs for appriasal on exceptional basis when employee performance is very low and non-acceptable requiring more frequent appraisals/actions according to the situation.

HR also determines the minimum and maximum time limits for the annual performance appraisal and usually it is anywhere between10 months (earliest) and14 months (latest).

Managers have to comply with these rules above regarding the frequency of their employees appraisal.
