In my sincere opinion a long my working experience fall downs maybe it´s the main problem, but provisional electrical installations in work areas in my my own judgement it´s a big problem to check or control...

Falls from heigts make up to1/3 of fatalitis on construction sites. By low scaffolding must be done by competent person and from experience this is not the case most of the time.
Communication factors which eventualy allows the construction worker to make decions on his own.This is due to the expert persons not being available on construction sites to address the employees issues immediately.Therefore we have many non conformances against a company because of poor management visibility to address workers needs.
Fall from height mostly material fall is very common in construction industry and the basic root causes is to missing of toe boards on platforms improper house keepings etc. Mostly50% of near miss incidents records in the shape of material fall.........
negligence.people who put on PPE think they are over protected and turn to neglect basic safety rules.e.g someone climbing a ladder wont ask another person to be at the bottom of the ladder in case it slips.he is relying on his PPE that he wont be hurt should he fall.
It never ceases to amaze me the number of persons who's first reaction is to say PPE. If employees are not following the site rules somewhere along the management and training route - youve failed! It may also be an indication that site management are not interested in safety - and therein is my answer to the problem - the lack of knowledge support and leadership in environmental and occupational health and safety in construction. my own concern is the poor control of the logistics / store department who IMO currently are a law unto themselves - from procurment to tool control, storage of materials to hazardous stores most dont have a clue of what is required - and what their responsibilities are. This produces hazards, risk of injury / fire / ill health etc. it also is very costly
Construction safety is a major concern as construction sites could be adjudged as the most dangerous place of work. This is mainly as a result of heavy machines and heavy human traffic on site. A multiplier effect of potential high accident occurrence due to the high frequency of interaction that may lead to impacts that are fatal. Consequently training and retraining to enhance competence on the SOP of these machines as well as implementation of policies that creates conducive environment for safe operations and or positive safety culture.
Hazards are everywhere, even if you are inside home watching TV. The real problem is the mind set and attitude of Management/ Workers. Everyone enter into the construction field with proper training about importance of PPE's and Safe System of Work. Most of the hazards including Fall from Height, Confined Space Hazards etc could be easily mitigated via JSA, PTW, Supervision and so on. So if the Safety First Attitude is there then definetely identifying these hazards and mitigating them is no big deal.
Alot of workers on site fail to wear their PERSONAL PROTECTIVE EQUIPMENT and find it hard to adhere to Safety precautions like proper positioning of scaffolds, use of belts at heights.
