Preparing your work by writing down your  objectives, identify the activties that needs to be done to complete your work,  identify resources required, identify other persons involve to complete the task, set timelines and target dates and to work your plans means to follow and try to complete all activties related to your plan ( It would be useless if you plan it out but never accomplished it )

From my point of view it is:

* Work your plan:- Know how to get satisfy work team.

Planning your work simply means be clear of your goals and plan accordingly, keeping in the mind the circumstanaces so that you are prepared of facing consequences or delays if occour and how to react to them

its very simple. Plan ur work means that priortise ur work and plan that how u r going to do the job/work.

Whereas work ur plan means   u should do each work according to plan. It can also like that u should reassess u plan that whether it is  practicable or  not.
