I think the basic problem, is the lack of respect for the employee. When the leadership doesn't consider the employees as worthy then it's all gone. You cant' expect then from the employees'side, loyalty or increased productivity or anything else. They will look for another job.
There can be many reasons :
1) High expectations.
2) Less involvement of management.
3) No proper career growth plan.
4) No proper HR department.
These are very few to mention here.
the main reason is loosing focus of the main goal the company is trying to achieve.
being involved in two many projects at the same time,unable to concentrate and focus,not setting attainable target will get anyone or any company disoriented and loose focus and control.
Like we need fresh air to breath for good health; working brain has the thirst for trying something new and when this quench is minimised it starts exploring new areas. Therefore to retain talent and working brain the organisation must have ample focus and opportunities for them to try new things and to learn new things.

Because they don't keep them motivated to work with them. there are certain reasons for loosing talent, a few can be:
- Competitors seems better employer / offer better compensation
- Employee's expectations are too much or inappropriate to be fulfilled
- Lack of career growth plan
- Behaviour of immediate boss
- Lack of employees' needs assesement etc etc.
Among other reasons for brain drain, reason s of importance include the attitude of the immediate boss, working atmosphere & career path.
Lack of opportunites
Poor Reward System
Political Games
Poor working Environment and someothers
but in my opinion Poor Reward System is the base of all these things
Poor leadership
not good working environment
There are a myriad of reasons but few of the culprits are;
1. lack of career development
2. ineffective leadership
3. indifference to the concerns of employees
