We are currently experiencing this at my company on a very large scale. There s mainly2 reasons for this. Firstly, management are working against their employees, meaning it is turning into a one man show. The employees are not feeling appreciated anymore as no inputs are required from them...Secondly respect for one another has escaped all of us. People are being mis treated on all levels, thus a work place that should feel like home(for the time you spend there) it is now starting to feel like corporal punishment. This destroys moral and positivity among employees and is like a virus. Spreading fast...

I think the basic problem, is the lack of respect for the employee. When the leadership doesn't consider the employees as worthy then it's all gone. You cant' expect then from the employees'side,  loyalty or increased productivity or anything else. They will look for another job.

There can be many reasons :

1) High expectations.

2) Less involvement of management.

3) No proper career growth plan.

4) No proper HR department.

These are very few to mention here.

the main reason is loosing focus of the main goal the company is trying to achieve.

being involved in two many projects at the same time, unable to concentrate and focus, not setting attainable target will get anyone or any company disoriented and loose focus and control.

Like we need fresh air to breath for good health;  working brain has the thirst for trying something new and when this quench is minimised it starts exploring new areas. Therefore to retain talent and working brain the organisation  must have ample focus and opportunities for them to try new things and to learn new things.
