Leaders can sometimes fail because they simply lose touch with their followers. It is often the followers that honour you with the position of leader, so always be aware of their wishes/ needs. Be visible. Don't make yourself inaccessible. As a leader, walk the "shop floor", wherever that may be. If the followers wishes cannot be fulfilled, take the time to explain why. Offer opportunities for training and advancement to others, without fear of compromising your own position. There are managers and there are leaders. Leaders pioneer without fear. Managers manage with what they have.
Not just having followers make a leader. A leader is that one who have influence at all levels and have the power of personality that make others to get things done in effective and efficient way
Leader is a good listener and learner and provides space for others to participate and values their contribution. Sets examples to be followed and relinquishes the position to take new challenges.
To be a leader need to know what is the aim of being ? is not only finish your tasks but you know the ways to deal with other persons inside and outside the company , finally you need to have communications skills is the basic concept of being leader and is to know when to speak and take action and when to make it pass.
In the exact meaning of a leader (from the verb lead), it implies there should be followers .. Otherwise, how would the act of leading happen if there are no people following. But this is not everything for sure !!
The other thing, while we all think of leaders in the positive way, we should know that there leaders who lead to failure and catastrophes .. and we have very clear examples in old and modern history that you can remember without me quoting names .... Even, we can say that the devil (Satan) is the greatest leader ... to bad things and disasterous end (from a religion viewpoint)
Now, for the supposedly (good leaders) who still fail. The failure could be from the leader side, the followers side, or for the lack of harmony/synergy between the two.
Let me quote the example of a "maestro" leading an orchestra. You need both (the maestro and the musicians) to be on the same wavelength to produce an excellent performance, Otherwise, you may get a lousy thing. The maestro in this (lousy) case is still a leader, but a lousy leader and the overall result is failure !!

Being integral to timeless international value system, having followers and developing new leaders identefy good leaders. Leading followers down the clif and not developing new leaders signify poor leadership.
Addin to the above, leaders fail if they don't have a winning vision (from thier followers point of view) or if they fail to effectively communicate their vision to the subject community.
I think a leader fails when he starts to think and act like that he is the only one and only he can do.

I beleive the leader fails when he start to think in vanity,conceit way
