How do we add a new name in already ticketed booking?

No airline allows you to add a new name to already ticketed booking. You will have to make a new booking with the current fare in the market and request airlines to link both the PNR's. If the existing ticket is within the void period you can void and make new reservation with all the names together. However, you risk in ending up paying a higher fare.
