Can you advice me with simple ideas in designing a survey using Microsoft office Suites?

what are the possible options and available solutions to design my survey, using MS Office applications like Word,....??? My prospective plans to rank the future job is to evaluate my performance at work, and making a valuable comparison to my previous career performances. so, you could help me in this.......

A survey is a reliable method to compile, compare and analyze data. Using Microsoft Excel, you can create a survey to distribute via email, over the Internet or on your website. Adding form controls to your Microsoft Excel spreadsheet can decrease errors and increase productivity when creating a survey. Microsoft Excel offers several form controls to create a survey, including option buttons, check boxes, list boxes and text boxes. The information that you include in the form controls can assist the end-user and limit the information entered in your survey

Actually Excel can give you great survey form which can be used either electronically by mails or as printed form filled manually.
you can put all your questions and ask your customer to put the rank (Ex: from1 to7) for each point.
then after you can collect your data and analyze to get the average for all customers; also you can plot the final results on a chart with base line so you can figure out the strengths and week points.

If you have Sharepoint, you can create a survey list.
You may also use Infopath and your back-end or data storage could be a Shapoint list or MS Access database.

Open a new, blank document in Office Word2007, and type a title and any other heading information that you need, such as your name, the date, or the name of the survey.
Also, if people will be taking the survey on their own, add instructions explaining how to fill out the form.
Type your survey questions and the multiple-choice answers.
Tip: On the Home tab, click the Numbering button to automatically number questions and choices.
By default, when you type a number followed by a period and space, Office Word2007 starts numbering for you.
Tip: Try using a table like the example below.
By using tables, you can fit your questionnaire onto fewer pages.
To create a table, on the Insert tab, click Table, and then choose how you want to insert the table.
Then, save and print it.

If you need to create a survey, form, or questionnaire, Office 365 has you covered.

You can use Excel Online to create a survey with a shareable link that saves responses in a back-end spreadsheet, allowing you to analyze your data however you wish.

The video above will show you how to create a survey, starting from OneDrive for Business, but you can also use your personal OneDrive to create a survey. To create your survey:

Click the new button and choose Excel survey.

Enter a survey title. This is for internal use, and will store the responses, so name it however you prefer.

In the Edit Survey window that appears, enter a title and description for the survey.

Click into the Enter your first question here field.

Enter a survey Question, and an optional subtitle.

Choose a Response Type:

Text: for short written responses

Paragraph text: for longer written responses

Number: for numerical responses

Date: for answers you can sort by date

Time: for answers you can sort by time

Yes/No: for yes or no responses

Choice: for a drop-down menu the survey taker can choose from

Check the Required box if the question is not optional.

Enter a Default Answer that will be pre-filled for the survey taker.

Click Done.

To add more questions, click  + Add New Question and repeat steps 5-10.

Once you’ve completed adding questions, click Save and View.

Review your survey, and click Share Survey to get your sharing link.

You also may choose to view and test your survey, using the sharing link, before you distribute it to others. This will help verify that the survey is working correctly and the questions will collect the type of answers you need.

When others take your survey, their responses will be stored in your Excel Online spreadsheet. You can do whatever you want with the data, and it’s a good practice to consider your end goals before you create the survey, as it may affect how you write and structure the questions.
