Blaming culture. 

not having the buy-in from both top management and employees. 

No planning for the whole initiative. 

No linkage for the initiative with other initiatives in the organizations. 

Not to consider all stakeholder expectations when conducting the initiative. 

Not developing required KPIs to measure the effeciency and effecteviness of the initiative while conducting it and after.
