My humble experience says; there's no differences between them, in the practical way.

But what science says; a Therapist has a psychology degree and he works in his clinic, but the Counselor has a counseling degree and he works in a specific institution, such like; school or company.

The significance differences are in term of1) type of the clients and2) technique used.
In counselling setting, the clients ranging from normal people who experience day to day stress, or dealing with certain decision making dillema and the technique used can be as simple as Rogerian.
Where as, therapy is inclusive of more serious psychological problem and requires deeper technique such as CBT as an example.
Other differences would be the duration/ times of the whole counselling sessions and therapy sessions, also maybe the fees, depending on the technique and the psychological inventories used along the process
