I will request him to speak loudly for better understanding.

If this problem is faced by majority, ask to keep the notes digitally available with hands on training. The best way is to get the trainer changed as lots of efforts and time is invested by an organization to schedule trainings.

I will say to him nicely and would say to him, ''Sorry sir i can not understand you clearly, can you please repeat again?'' simple is that , and if you still cant understand him after multiple times you told him to repeat then tell him you can not understand him, he might/will get offended and might get angry, but hey if you will not understand what he is saying then how you are going to learn and perform?

It depends on how many (%) of the participants having the same problem...... if a large no. of participants are unbale to understand the accent of the trainer, he must not be in his field ( i am sure, it is not the case).
If he is having a sore throat or any other disease, one can postpone the session.......
Beside this, if you are the only one (or a few of particiants) having the problem, You need to be climatized with such accent before joining this session, and may also need to improve your listening skills.
Keep asking until you understand. That's what are you paying for.
Trainer should have the capabilities to explain in simple terms, if a request is coming from the participants. You can cordially respond with a situation and ask him for solutions in simple terms.
There will also be "Feed back" reports after training in which you are at liberty to express your views, whatever you think appropriate and apt in a polite language.
