It depends on the project size, duty, budget, and responsibility. Project Manager follows a tactical approach to manage the project, while project leader follows a strategic approach to lead the project resources by allocating and optimizing them between many related sub projects.

I agree with Bassam, a leader will take the time to tell you what to do and explain the objective of the project whereas the manager will give orders (demands). A leader has patience to solve any obstacles, understanding of different peoples and their abilities.
