Need to deploy project monitoring tools, such as; Earned Value Management, Employee Performance appraisals, PERT, Critical Path analysis, and others.

To my experience, beside we apply best project monitoring tools, keeping close eye on planning and keeping every thing well documented, etc, etc.. If there is Lackness found in governance then sooner or later the project goes into deep trouble.

In my opinion, the following conditions may forecast EWS of project going red: 1. No stakeholder involvement and active participation 2. Team members lack requisite knowledge and/or skills and weak commitment 3. Lack of documented requirements and success criteria 4. No change control process in place 5. Ineffective schedule planning and communication 6. Often accruing resource allocation conflict 7. Not properly defined business case for the project and top management commitment When we see these conditions, and address them, we may successfully close the project to the best satisfaction of the stakeholders.
