I believe training needs of an employee can be identified from the Key Performance Indicators of his/her role in the team.
Training needs can be assessed on how a person performs, what is expected of his/her performance and what he/she needs to do to perform better.
E.g - a data entry operator can be given additional training in excel, macros to help him/her perform better and faster without many errors

The training needs are usually identified at the time of Annual assessment by management where the employee honestly communicates his/her training needs.
It is up to the management to accept or reject.
However if the management decides to support the training needs then they should properly assess it at their level.
The way to do is to look at the skills required to carry out this job efficiently.
Then list out the skills the individual posseses in fair quality.
Next step is to identify the gaps in his/her skills and target them for training over a period of time prioritising the most needed first and so on.

The need for training to the employees can be identified when the employees are not meeting their required performance level by the supervisor.
Some times to training required when new jobs or functions to be performed.

Measuring employees performance periodically i.e Monthly,Quartery is the best way to identify whether he/she needs the effective training towards process and particular task.
Assessment should be done on every task and to provide advance training.
