A- Could not gain profits and could not maintain his company market share.

B- Could not unite the team, or high level of turnover at his era.

C- Could not achieve the Goal.

D- Inappropriate Organization culture, environment, administrative procedures.

E- He can add or do more, and he had enough.

F- Mix of them, but at this case Either he chose a wrong company, time, career to work in Or he is not experienced enough to work in this company.

I choose answer "F" and I say he might have chosen the right company and team, but he/she is just not capable to do the job assigned to him/her. Some bad managers are bad even though they might have good teams and good companies

I'd choose "F" Mix of them". No matter how bad the firm's situation is a good leader can always rise in every setback along his way. However, provided that he's employed in a good and successful company and staff but without or lack of enough knowledge and skills would not put him to success but rather failure.

I strongly select 'B' although I don't think any of these points could or should end their 'journey'.A leader is only defined as such by their ability to lead people - which is the most vital organ of any business. Being able to unite a team and stabilise a workforce - especially one that is disenfranchised - is something that derives from clear and decisive leadership. If one can't achieve that while in post as a CEO or Managing Director that cast serious doubt on their ability to engage and lead.

The differentiation has to be made between a leader and a senior manager / director.
