Where are quality policies and standards established in?

Quality policy and Quality Standards are two different things.

Quality policy should be developed jointly by Management and Quality experts along the guidelines shown in the standards. It is a mandatory requirement in Standards and it should cover entire aspects of product quality to achieve customer satisfaction.

I am not sure if I got the question but policies and standards are come from standard bodies and by using them the qulity management plan will be issued which describes how project management team plans to meet quality requirements set for project.

Quality policies are established where products or services are developed from inception to production and marketing to delivery. Quality standards are established where products or services that where developed and offered are in conformance and consistent to what has been conceived as acceptable or specifically to standards established by internationally known institutions.
