The below informations shall be covered in the accident reports

2.DESCRIPTION OF INCIDENT TO INCLUDE WHAT HAPPENED AND ANY DAMAGE OR INJURY SUSTAINED

3   INCIDENT REVIEW/CONCLUSION OF INVESTIGATION

4. ROOT CAUSES

5. ACTION/S TAKEN TO PREVENT RE-OCCURRENCE

a. IMMEDIATE CORRECTIVE ACTIONS TAKEN

b. PREVENTIVE ACTION AND ANY ADDITIONAL ACTIONS/RECOMMENDATIONS

c. HOW WAS THE PREVENTIVE/ADDITIONAL ACTIONS/RECOMMENDATIONS COMMUNICATED OR IMPLEMENTED

d. REFERENCE and supporting evidance

Name of the person injured. Date & time of the incident. scene record. witness statement. Accident details. Direct cause of the accident. Root cause of the accident. control measure for stopping reoccurance.

1) Place and Incident of accident happened.

2)Number of persons injured/dead.

3)Damage to property or public.

4)Cause of accident.

5)Witness statement if any.

6)Corrective action taken.

Lessons Learned is very important. Also every report should investigate on the Preventive and Corrective actions.

In preparing the accident and incident report, the following should be considered;1) the details of the person involved and person reporting the accident includes time, date, location, other parties involve, followed by 2) classification of accident according to type, injury, factors causing the accident, lost time then 3) exact description of how accident had happen includes the immediate action that has been taken and lastly4) take the details of the witness.

What caused the accident, when did it happen, impact of the accident, people involved or mobile plant, damage to property or injuries, snap short of the accident scene.

a) identification of the date, time, injured people and reporting person

b) name and designation of witnesses

c) description of the accident from the witnesses perspective (collection of witness statements taken as as soon as possible after accident)

f) collection of all the evidences (pictures, etc...) and statement

g) records of question/answer to witnesses

h) copy of all operational documentations that can be relevant (shift schedules, training records, safety procedures etc....
