 large number of accidents happened each day in many industries all over the world, which result in injury to people, environment pollution and consequential reputation damage. By investigation into accidents, we can identify root causes of accidents and develop implement corrective actions, which prevent accidents from reoccurring. Accident root cause analysis method is introduced in this paper. By analyzing each causal factor of accidents, root causes are found, and corrective actions are developed due to root causes to prevent accidents from reoccurring. 

Identify the causal factor,which may be human error,enviromental factor or mechanical.

Gather data,infomation and securing of evidence.

Identify the possible root cause of the accident.

Come up with the corrective action to prevent the recurence.

Imlementation of the corrective measures to mitigate incase of recurrence. 

RCA Basic Method

• Define the problem.
• Gather information, data and evidence.
• Identify all issues and events that contributed to the problem
• Determine root causes.
• Identify recommendations for eliminating or mitigating the re-occurrence of problems or events.
• Implement the identified solutions.

For me Fault tree analysis  is definitely the best one
