As a manager, have to review and update procedures, expanding in "standardization" in processing for all types of operations, as this will avoid and minimize both mistakes and improvisation, in addition to conducting more staff awareness training courses. 

As a manager we have to be cautioned and move your steps. You have got a responsibility to manager and control and give your performance .Unfortunately, if it has happen by oversight try to fix yourself, because we all humans.

Another way could be that manager ought to allow enough space to team to express their ideas for any other better options to be mutually opted and to strengthen their commitment thereby reducing the risk of failure and opening room for learning for all.
