Nope. it will create mess. you must follow hierarchy and there must be procedure if your immediate manager is not responding your written request (email, letter etc) for 2 business days then you should go to one step up and if the situation is same you move one step ahead. but you should follow hierarchy first.
On certain occasion yes top management should mingle up with all employees to get exact idea of what is going on into the organization.

Not necessary. employees can have a full communication only to their own managers assigned to command and to decide whatever important things for business matters. If they belong to middle level their scope of communication is not necessary beyond to it , unless important and legitimate with permission of the managers in charge.
