Employee ----> Team Leader-----> Supervisor------> Department Manager------->CEO or COO----> GM------>.........

Nope. it will create mess. you must follow hierarchy and there must be  procedure if your immediate manager is not responding your written request (email, letter etc) for 2 business days then you should  go to one step up and if the situation is same you move one step ahead. but you should follow hierarchy first.

On certain occasion yes top management should mingle up with all employees to get exact idea of what is going on into the organization.

Access and communicate something nothing else, access means being accessible to them at any time, this is unacceptable, because there is a hierarchy in the organization must be followed, and the communication I do not mind it at specific times according to the Organization's policies and regulations.

Employees from all levels having full communication acess to mgt has both its own merits & demerits as weell. Therefore i believe that a balanced system should be put in place where employees from all levels should elect their own prefered representatives (reasonable number) who can have full communication acess with top mgt in view of representing their needs to the top mgt. Not necessary. employees can have a full communication only to their own managers assigned to command and to decide whatever important things for business matters. If they belong to middle level their scope of communication is not necessary beyond to it , unless important and legitimate with permission of the managers in charge.
