It is depends on the company maturity level:

1. Assign a task team to use suitable problem solving technique to overcome this problem.

Or:

1. you must revise your plan and looking for what you missed during planning period.

2. you needs to revise how match you organized your team/materials well according to your plan? 

3. If every thing is OK so you must know if everyone knew his responsibilities and duties the way you assigned for or not.

Thanks for invitation, In a very precis wording, the steps for solving a problem are as follows:

1- Define the problem correctly.

2- Searching for all possible problem solving alternatives. 

3- Investigating and studying all available alternative carefully and chose the optimum.

4- Implementing the chosen alternative and carefully following up results.

5- If the problem is solved and have positive results, take the necessary action to modify and correct the procedures accordingly.
