There are strategies you can use to avoid common pitfalls and hone your decision-making skills. Making better, faster decisions will help you take advantage of business opportunities and avoid pitfalls, I would say if we can take the decision based on the following criteria would give us good end:
1. Reframe the problem
2. Make evidence-based decisions.
3. Challenge the status quo.
4. Get an outside perspective...but trust yourself.
5. Develop an eye for risk.
6. Let go of past mistakes.
7. Be honest with yourself.
8. Be decisive.
You have to take the decisions on the available information and that time circumstances. Therefore, you can not say all the time you do right decisions but you have to take it as per your best knowledge and efforts. You have to make some assumptions then you move forward for decision making, Never afraid and do what you feel best as per your available knowledge and skills.
No , No one have right decision in all time , but when I try to get right decision we need to share our opinion and if I'm good manger or working by professional way should be call my assistant to get all department manger and we can discuss after that I tack the decision

No. Every manager is not perfect in making decision because of limited knowledge on the things or experience. However decision should always be fair to the company or anybody else.
