It should be D. As Project Office, is only true, if organisation has PMO. Project Team forms in planning stage.

Charter contains details on Sponsors.

Charter contains a very high level details on project and with high level of Risk/Issue. lot of items yet to be discover in Planning and Monitoring.

One key object of Charter is to assign a project manager and provide him Authorization to work. 

So, best possible answer is D.
