To err is human. It’s a fact of life. The reasons for making a big mistake are various and not all of the mistakes are done intentionally. But, what happens if a mistake made by one of your employees causes a major mess-up that will cost your company a fortune?
A mistake is a mistake. It does not have a size big or small. I would first remember before taking any action that I am a human and I can make mistakes, that will ease the action, the second thing is that I would not look at it as a mistake rather than to look at it as a lack of maybe understanding the problem or lack of problem solving skills.
I would listen to him to help him rather than listen to him to punish him. At the end; we are one team and if he fails I fail. I will deal with this mistake from a professional point of view and not from a personal anger aspect.. As long as no one died.. things can still be fixed.

Considering the Fact "No one is Perfect in the World" so the mistakes happen and we need to tackle it with patience and proficiently with the team member who did it. Do not take any negative action against the doer without getting his interpretations and he should be given chance for improvements.
Lastly, I recommend to document the incidence with the root cause and lesson learned from such mistake and archive it.

It is no use crying over spilt milk or whatever the saying goes. The most important thing is to learn from the mistake. Was the mistake due to an error in judgement? was it due to negligence? was it something that could have been easily avoided? Was it very basic or was it quite complicated? Many more reasoning can be made and the end result should always be "How it can be prevented next time" As for the employee, it depends on what grounds the mistake was made but it is always wise to remember a human can make mistakes and getting past them and coming with up solutions to avoid this is also part of being human.
