Output is the immediate result of any process. Outcome is the long term result. Planting One million trees: Output : One million plants. Outcome: One million trees resulting in more rain fall and less pollution.

output is what you produce in result of your input that doesn't measure your performance. 

Outcome measure your performance and achievemen in the result of your input.

Outputs tell the story of what you produced or your organization's activities. Output measures do not address the value or impact of your services for your clients. On the other hand, an outcome is the level of performance or achievement that occurred because of the activity or services your organization provided.

Output is a short term result of an process, where as outcome is the result after output. Suppose, you organized a conference of AIDS awareness. When the conference is done it is called output of the conference. But after the conference if participants starts making awareness on other people or they start any social movement it is called an outcome of that conference
