I think establishing safe place through the good designing of workplace and planning process, is the most important aspect because control measures that depend on human behaviors are not effective as administration and engineering controls

Raising the safety culture is the main reason of preventing accidents , the culture is for persons not places so the safe person will conduct to no accident
No doubt that the safe place is very important but not the first pirority
SOP which is made based on existing real activities by applying work permits for every work that will be carried out with high risk.
Sure safe place/condition. It's necessary to remove every danger from place/ambiental condition, and apply Personal Protective Equipment only when there isn't any other possibility. PPE could be obstacle woker, so it is better to delete most of dangers near him.

As per my view 'A committed organization that ensures a continually improving Safety Culture and a dedicated workforce development that considers Safety as a self-responsibility rather than a compliance obligation', is the most important aspect to prevent an accident as such a workforce will not work in an unsafe place/ condition and would promptly inform the management about it, whereby preventing the probability of accident at first place. More than anything it is the force of trust and reliability that a committed management transfers through its workforce that moulds the best QHSE culture
Even with people without safety-oriented minds, a safe work area will keep accedents propability down hence improving the accedent prevention.
Therfore, I think a safe work place/condition comes first
Safe Person, because unsafe place/condition can be identified by any person in the workplace and it can be rectified immediately. But an unsafe person needs to have behavioral safety culture inside of him to practice safety.
From my point of view and through All accident/incident investigation I went through both of them they are important.
You can't provide a safe working environment (Safe Condition) and neglecting the culture and behavior and vice versa.
So you have to start with the workplace by providing the required control measurement to keep the workplace safer meanwhile you have to work and the culture and behavior of the people by giving training awareness etc.
Once unsafe condition and unsafe act come together surely accident will happen.

because you can't manage people you can manage the operation or the process.
Both safe person and safe condition are important. Unsafe person contribute high percentage of accident.
