Decisions are core and needs to be designed to meet the goals and needs of the organisation. Facts are needed to be read before you take any decisions.
Failure to achieve this is an immature way of thinking.
It is simple the clear picture was obscured and hence the problems will be faced openly. Strategically speaking "Planning to fail, and Failing to Plan" is the situation now.
Being accountable for any mishap in the organisation is the forefront of action.
We have to be ready to have an alternative to work in this situation. Questioning ourselves and gaining an analysis on the scenario is key issue.
Have two teams ready to tackle the defficency and alter the problem to positive efffects with a lesser mark in failure of targets.
Own up to the mistake. Have another look at the objective. Ask the person(s) who own the project to kindly give feedback prior to making the last decision.
Everyone makes mistakes; own it and move forward. In this case, fix it.
