All the above four skills require you to practice them as much as possible. You should start from your typing speed, practicing at least one hour daily. There are many good typing softwares available (just google them). To improve medical terminology, there are many books and references available on the internet that you can get them either in soft copy or hard copy. Medical Dictionaries are also available (e.g. Stedman's medical dictionary is one of the good choice). English Grammar is the most important factor that needs to be followed if you want to be a Professional in medical transcription. Many doctors often make mistakes while dictating so YOU are responsible to correct their grammatical errors. Listening skills can be improved by practicing sample audio files as much as possible. More you practice them and type the more you get accuracy in your listening. Do not restrict yourself to just one accented doctor, but practice in as much as possible. You should take start from the easiest and slowest audio file. Once you havea complete command on all the above points, you can be a professional transcriptionist, but the condition is that you must take interest and devote your time.
I would also suggest that you do not restrict yourself on a single speciality, but to practice every speciality so that when it comes job, you are not rejected.
