What is the difference between basic, root and systemic causes of incident?

Basic causes are sometimes called immediate causes.

  • basic causes is the direct contributing factor to the incident, a basic reason that motivate people do the unsafe acts or make an unsafe condition
  • root causes is the factor behind the contributing factor, usually it already exist for a long time but since it did not directly make an incident/ near miss, people did not aware with the consequences
  • systemic cause is one of the last category of the root cause. sometimes the system is correct but the doer (poople) do wrong/mistakes. or even system and people already correct but the equipment (physical/ material) that bad.
  • systemic cause can only be known after review if the policy, guideline, procedures  etc is potentially contribute to the incident/ near miss
