We are planning to run an employee satisfaction survey for our employees. We have various section in it like work profile, peer-boss relation, job rotation/job role etc. any suggestions further.

very much depends on the purpose behind this survey and the real intentions behind it. If it is just a formality mandated by law, direct request from internal/external auditors, quality management office, ... etc. then these people are often the best people to consult with and explore their needs/expectations from such a survey and what they need/want/expect to see in it.
